How to Make a Referral
“I wish we would have contacted you sooner.” That is something we hear all too often from families once their loved one has started our hospice services.
Who can make a referral?
A referral can be made by:
- The Patient
- Families
- Healthcare Providers and Physicians
The referral starts with a free consultation with our admissions care consultant. The consultation can be in person or via phone call. Our trained admission nurses will help guide patients and families through the end-of-life conversation and answer any questions they may have. They will go into greater detail about the benefits of hospice services for the patient and family.
Our admission nurse will then meet with the patient and family wherever they call home, whether that be the patient’s home, a nursing facility, or hospital, to evaluate the patient’s current condition and review medical records. Once the evaluation is complete, the admission team members collaborate with our hospice physician and the patient’s primary care doctor to determine if the patient meets the criteria for Hospice Care.
If the patient would not meet the criteria for starting hospice services, we offer other options such as a Palliative Care consultation or our InTouch Program.
