There may be times when you or your loved one cannot continue to receive hospice care where you call home. EverHeart Hospice created the Hospice Care Center with a home-like environment designed for short-term General Inpatient Care.

Our care team offers a supportive presence with 24-hour nursing care, with a focus on providing comfort, medication adjustments, and providing education and support to transition patients on the next phase of their journey.

Our Hospice Care Center provides:

  • Short-term General Inpatient Care to manage uncontrollable symptoms, adjust medications, and provide education and support to transition patients on the next phase of their journey.
  • Support and education to patients and families on disease processes and medications. Support is offered to patients and families through 24-hour nursing care with supportive visits from our collaborative care team of hospice aides, social workers, chaplains, music therapists, bereavement support, and volunteers.
  • Frequent visits by the hospice medical director or hospice nurse practitioner.

Symptom Management

Our patients receive a full scope of non-curative medical and support services for their life-limiting illness. Managing your pain and symptoms is integral in the care we provide at our Hospice Care Center. Below are the ways we address the following types of symptoms:

Physical – The Hospice Care Center staff provides frequent assessments, interventions, and evaluations around the clock to ensure the patient’s goals are being met with pharmacological or non-pharmacological interventions. Music Therapy can also be provided as an alternate therapy for pain and symptom management.

Psychosocial – We encourage family and friends to offer support by visiting them during their stay. Supportive visits from the EverHeart Hospice team include social workers, chaplains, music therapists, volunteers, and the bereavement coordinator and are individualized according to the patient’s needs and desires. It may not always be about finding the right medication, but finding the right hand to hold.

Spiritual- Spiritual wellbeing for the body and mind is a crucial piece of the puzzle that may need attention. Our hospice chaplains make supportive visits based on individualized needs to provide support, prayer, or a listening ear. Our bereavement coordinator is a call away for patients or families to provide support, encouragement, understanding, and counseling.

How Your Care is Covered

The scope of hospice services is defined by the Medicare Hospice Benefit. Your hospice benefit covers care for your terminal illness and related conditions. EverHeart Hospice accepts Medicare and Medicaid coverage as payment for Hospice Care Center services. Most private insurances also cover these services. EverHeart Hospice will never turn anyone away due to the inability to pay. Please see Paying for Care for more information on how your hospice services are covered.

We recognize how difficult it may be to leave your home or leave your loved one in a new and unfamiliar setting. We understand communication is vital to the continuity of care. We want to reassure you that we are available 24/7 by phone, and our door is always open for a visit with your loved one.
