InTouch

Our InTouch Program is a non-medical service we provide for individuals who may not meet the criteria for Hospice Care or those not quite ready to start hospice services. We understand everyone may be at a different point during their life-limiting illness, and we want to be of assistance to you at every stage of your journey. 

During your journey, we offer support from our trained care consultants, education, and resources for you and your family. InTouch is available at no cost to you, does not need a physician referral, and is available wherever you call home.

A few examples of the support we offer:

• Routine communication with our care consultants.
• Assistance with community resources available to you, including food pantries, support groups, transportation services, free legal services, and information on Medicare/Medicaid/Private Insurance.

Over the next six months, we would like to contact you to see how things are going and help answer any questions or address any needs that arise. If you have any of the following symptoms, such as weight loss, poor appetite, swallowing difficulties, increased weakness, or fatigue, please don’t hesitate to call us.
